• Monthly Premium: $0 per month
• Network Details: This plan includes in-network and out-of-network benefits. Be advised that out-of-network doctors may have higher co-pays – check that the doctor will bill out of network before scheduling your appointment.
• The UCard is the card you use for anything the plan covers.

• The Formulary has a complete list of medications covered by your insurance.
• Mail Orders Prescription option is available through OptumRx or by calling 1-877-889-6358.  You can also download the prescription mail form
• In-Network Pharmacy Search

• There is a $495 annual deductible on tier 3-5 covered prescriptions.  Below are pricing at a preferred pharmacy.
  • Tier 1 Preferred Generic- $0
  • Tier 2 Generic- $14
  • Tier 3 Prefered Brand- $47
  • Tier 3 Insulin - $35 (no deductible)
  • Tier 4 Non-preferred Brand- $100
  • Tier 5 Specialty- 27%
• In 2025 there is a $2,000 annual out of pocket maximum on covered medications. 
• There is a new Medicare Prescription Payment Plan (MPPP) option that allows you to spread your cost‑sharing over the remaining months of the plan year.  

Additional Benefits:

• Free gym membership is offered at participating gyms.  You can find gyms in your area with Renew Active.
• Over-the-Counter Benefits - Your plan has $50 of over-the-counter benefits replenished every three months.    
  • The over-the-counter benefits can be used at Participating Retailers
  • Alternatively, you can purchase items using the plan's catalog.
