• There is a $300 annual deductible on Tier 3-5 medications. 
  • Medication costs are based on the tier of the prescription.

• In 2025 there is a $2,000 annual out of pocket maximum on covered medications. 

• There is a new Medicare Prescription Payment Plan (MPPP) option that allows you to spread your cost‑sharing over the remaining months of the plan year.  

 

Additional Benefits:

• $200 towards gym membership, home fitness, or fitness equipment.  
• Over-the-Counter Benefits - Your plan has $65 of over-the-counter benefits replenished every three months.    
• The Braven Smart Card can be used for OTC benefits, Flex Benefit, Rewards and Incentives.
