Prescription Coverage

Below are online search tools for your Prescription Drug Benefits:

• The Formulary has a complete list of medications covered by your insurance.
• Mail Orders Prescription option is available through CenterWell Pharmacy or by calling 844-628-2720. 
• In-Network Pharmacy

There is a $590 annual deductible on tier 3-5 medications prior to being covered. 

• After the deductible medication costs are based on the tier of the prescription.

    • Tier 2 Generic- $5
    • Tier 3 Prefered Brand- $47
    • Tier 3- Covered Insulin- $35 (no deductible)

    • Tier 5 Specialty- 25%
• In 2025 there is a $2,000 annual out of pocket maximum on covered medications. 
• There is a new Medicare Prescription Payment Plan (MPPP) option that allows you to spread your cost‑sharing over the remaining months of the plan year.
