• Appoint a Representative to speak on your behalf.
• There is a $590 annual deductible prior to medications being covered. 
  • After the deductible medication costs are based on the tier of the prescription.
  • Tier 1 Preferred Generic- $5
  • Tier 2 Generic- $10
  • Tier 3 Prefered Brand- 19%
  • Tier 4 Non-preferred Brand- 32%
  • Tier 5 Specialty- 25%

• In 2025 there is a $2,000 annual out of pocket maximum on covered medications. 

• There is a new Medicare prescription payment plan (MPPP) option that allows you to spread your cost‑sharing over the remaining months of the plan year.
